Advance Care Planning Implementation Guide
The National Ageing Research Institute (NARI) is undertaking a project, funded by Decision Assist, which aims to develop an advance care planning implementation guide for aged care providers in community and residential care settings.
If you are a person working in the community or residential aged care sector, you are invited to complete a survey exploring how advance care planning is implemented and discussed within your organisation. The answers they get from this survey will assist them in developing the advance care planning implementation guide. The survey will take you approximately 15 minutes to complete. Please complete the survey by Friday 7th of July 2017.
