Grant Opportunity: Support at Home Program – Thin Markets (rural, remote and specialised) 2025-26 (Round 2)

The second grant opportunity under the Support at Home Thin Markets Grants program -Support at Home Thin Markets (rural, remote and specialised) 2025-26 (Round 2) Grant Opportunity - is open for applications. Applications close 14 January 2026.

The intended objective is to provide flexible funding to meet appropriate business expenses, to enable providers in thin markets with high costs to set reasonable prices for Support at Home services.

This grant opportunity provides flexible funding to support service delivery for Support at Home providers operating in thin markets (where there is a gap between the aged care needs of older people and services available.)

Support at Home providers can be eligible for this grant if they meet either:

1. the geographic eligibility classified as Modified Monash (MM) areas 3 to 7; and/or

2. specialised service delivery eligibility criteria

The Centre for Cultural Diversity in Ageing would like to highlight the Specialisation Verification eligibility criteria for this grant opportunity. Support at Home providers who have an approved Specialisation Verification such as CALD, or who have applied for Specialisation Verification, can apply for this grant opportunity.

Note: Specialisation Verification application for outlet must be submitted by the grant closing date and approved by 20 February 2026 to be eligible for the grant.

Funding under the Specialisation Verification criteria is approx. $68,000 per 100 Support at Home Packages.

The Centre can provide support to aged care providers with Specialisation Verification CALD applications through the Cultural & Linguistic – Diversity Mentoring Program.
